Healthcare Comes Home for the Matonti Family

Meet Anna Matonti.

Anna and her husband Giuseppe ‘Joe’ are first generation Italian immigrants who came to Albuquerque, New Mexico in 1969 and started a beautiful family. Their daughters Rosa and Maria still live nearby – Maria lives next door. Unfortunately, in 2006, Anna became unexpectedly paralyzed and was hospitalized 11 times. Almost ten years later, she hasn’t been back, thanks to an innovative model of care in New Mexico.

The healthcare team from Presbyterian Health Services’ Healthcare at Home program in Albuquerque visits Anna and her family caregivers regularly, providing home-based healthcare and long-term services and supports that are essential to helping seniors, like Anna, age successfully, wherever they choose. To address her changing needs, her care team can increase the intensity of services when necessary.

We believe Anna’s story is another example of how using more home and community-based healthcare models can be safer, more effective and more affordable than traditional hospital-based care. This program has shown savings of 19 percent with better patient outcomes versus traditional care.

Patient-centered care like this is essential to helping seniors across the country age successfully, wherever they choose. For Anna, that’s at home with her family.
