About Models of Care
Since 2006, West Health, through the Gary and Mary West Foundation, has committed over $250 million through 650 grants to establish and bolster programs and research focused on aging and reducing healthcare costs.
The organization has played a pivotal role in shaping and implementing California’s pioneering Master Plan for Aging and has been a driving force behind the expansion of geriatric emergency departments and the proliferation of home-based care across the United States.
West Health has also spearheaded the development of several groundbreaking models of care that have become exemplary standards nationwide, including:
- The Gary and Mary West Senior Emergency Care Unit at UC San Diego Health, the first nationally accredited geriatric emergency department in California. This model has been adopted by over 300 programs across the country, including nearly all emergency rooms in San Diego County.
- Gary and Mary West PACE in San Marcos, Calif., a Program of All-inclusive Care for the Elderly (PACE) that provides comprehensive care, nutrition, rehabilitation, transportation and support, empowering seniors to maintain health and reside in their own homes.
